Chronic Care Management

For patients with two or more chronic conditions, the Chronic Care Management (CCM) program helps improve the coordination of healthcare with an emphasis on overall well-being. This program assigns a nurse to each participant to answer questions regarding medications and health conditions as well as provide education and resources to help manage chronic conditions and reduce emergency department visits.
