In extension of our Diabetes Self-Management program, our Coordinated Care team can help those with type 1 or type 2 diabetes get set up with a professional who will provide education on nutrition, medication, monitoring, activity, healthy coping, and problem solving.
Our Diabetes Self-Management Education program is accredited by the Association of Diabetes Care & Education Specialist (ADCES).
